Bedside Ultrasound in the Diagnosis and Treatment of Children with Respiratory Difficulty Following Cardiac Surgery

Abstract: Many children frequently fail extubation or require a prolonged period of positive pressure ventilation support after cardiac surgery. Pediatric cardiac Intensive Care Unit (PCICU) ultrasound (US) stands as a simple, basic bedside tool that can be performed by trained intensivists for the diagnosis with immediate implication on therapeutic decisions in multiple scenarios that physicians may face in PCICU. Bedside US is widely available, convenient, and inexpensive. This educational article based mainly on our … Show more

“…US-PPT technique provides a simple bedside and feasible tool to identify the tube position without need to utilise multiple ionising radiation or transport to the medical imaging department for fluoroscopic positioning. 5,6 Ultrasound allows us to follow the trail of feeding tube by using the landmarks. The data related to US-PPT placement in paediatric ICU are still insufficient and not well described.…”

“…5 Ultrasound-guided procedures have increased in the last 30 years in ICU. 4,6 The data related to US-PPT (ultrasound-guided post-pyloric feeding tube insertion) placement in paediatric ICU are insufficient. 4 Ultrasound has several advantages such as being real-time bedside examination that does not expose the patient to radiation or mobilisation.…”

“…16 Recently, bedside laryngeal ultrasonography has been used as an initial assessment tool for post-operative vocal cord function in the paediatric ICU. 24,25 It helps establish an early diagnosis, which can help us plan an appropriate management strategy. Furthermore, a speech-language pathologist should perform a formal feeding evaluation and rehabilitation once the diagnosis of VCP has been made.…”
