Behçet's disease and cardiac arrhythmia

Abstract: Only few cases of cardiac conduction disturbances and arrhythmias have been reported in Behçet's disease. We recently observed the case of a 16-year-old woman with Behçet's disease in whom cardiac arrhythmia became the main clinical symptom. This observation and a review of the literature led us to the conclusion that arrhythmia could represent the clinical manifestation of an underlying myocarditis due to Behçet's disease and can be regarded as a feature of cardiac involvement of the disease.

“…Albeit rare, cardiac involvement in Behç et's disease may present with arrhythmias, such as paroxysmal atrial tachycardia, premature ventricular beat, or conduction disturbances. 33,[38][39][40] In this series, complete heart block occurred in one patient preoperatively and two postoperatively, necessitating implantation of a permanent pacemaker. The real reason of heart rhythm disturbance was not clarified and might result from inflammation of the conduction system.…”

“…A few case reports have indicated pleural effusion due to primary vasculitis of the pleura (6). Cardiovascular involvement, including coronary artery disease, pericarditis, myocarditis, endocarditis, arrhythmia and valvular dysfunction, have been reported in <10% of patients with Behçet's disease (9)(10)(11). Pericardial involvement, a common cardiac manifestation with up to 40% cardiac involvement, may be associated with vasculitis of the pericardium or SVC thrombosis (6,10).…”
