Background: In 2008, a nationwide standardized and systematic skin cancer screening (SCS) service, which is globally unique, was implemented in Germany. We aimed to provide current data on SCS use, to explore barriers to SCS usage, and to identify population groups with lower rates of SCS use. Methods: We analyzed data from 2,635 participants (18-45 years) in the National Cancer Aid Monitoring on Sunbed Use. Data on SCS use, barriers to SCS, and sociodemographic characteristics were obtained. Chi-square tests and logistic regression analyses were performed to analyze the data. Data was weighted by age, sex, educational level, and federal state. Results: In total, 39.0% of participants reported having been screened for skin cancer at least once in their lifetime. The subjective importance of different barriers varied depending on the participants' educational level. SCS use was negatively associated with male sex (odds ratio (OR) = 0.63, p < 0.001), low level of education (OR = 0.83, not significant), immigrant background (OR = 0.63; p < 0.001), and having no employment. Conclusion: Although the SCS is part of the regular healthcare services offered in Germany, our data showed lower usage among certain population groups. Barriers relevant for these groups should be considered when developing measures to increase SCS use.

Кулакова» Минздрава России, Москва, Россия РЕЗЮМЕ Проблема ранней диагностики злокачественных новообразований (ЗНО) кожи становится все более актуальной по причине роста заболеваемости примерно на 30% за последние 10 лет [1]. В связи с ростом заболеваемости и недостаточной эффективности скрининга ЗНО необходимо повышать информированность врачей неонкологических специальностей о признаках ЗНО кожи путем обучения навыкам выявления подозрительных новообразований. Цель исследования -повышение уровня информированности специалистов первичного звена для ранней диагностики опухолей кожи по клиническим и дерматоскопическим данным. Материал и методы. С этой целью был проведен 3-часовой мастер-класс по диагностике и дифференциальной диагностике новообразований кожи для студентов и врачей неонкологических специальностей, в ходе которого в три этапа демонстрировались фотографические изображения доброкачественных и ЗНО. На I этапе оценивали базовый уровень знаний участников исследования. После I этапа была прочитана лекция с изложением клинических признаков невусов, себорейных кератом, базально-клеточного рака кожи, меланомы. Затем участникам была предложена вторая лекция, во время которой были продемонстрированы основы структурного анализа при дерматоскопической диагностике новообразований кожи. Результаты. Повышение информированности студентов старших курсов медицинских вузов и врачей неонкологических специальностей является эффективным инструментом раннего выявления опухолей кожи. Однако использование дерматоскопии для ранней диагностики меланомы должно быть в компетенции узкого специалиста, владеющего методом в совершенстве. Кратковременное обучение помогает врачам неонкологических специальностей в диагностике подозрительных новообразований кожи с последующим направлением пациентов в специализированные центры.Ключевые слова: телемедицинские технологии, дерматоскопия, злокачественные новообразования кожи, врачи первичного звена, ранняя диагностика.

BACKGROUND After a pilot study on skin cancer screening was performed between 2003 and 2004 in Schleswig‐Holstein, Germany, the country implemented what to the authors' knowledge is the first nationwide skin cancer screening program in the world in 2008. The objective of the current study was to provide details regarding mortality trends in Schleswig‐Holstein and Germany in relation to the screening. METHODS Annual age‐standardized mortality rates for skin melanoma (using the 10th Revision of the International Statistical Classification of Diseases and Related Health Problems [ICD‐10] code C43) and malignant neoplasms of ill‐defined, secondary, and unspecified sites (ICD‐10 code C76‐C80) were analyzed. The European Standard population was used for age standardization. A bias analysis was performed to estimate the number of skin melanoma deaths that may have been incorrectly counted as ICD‐10 code C76‐C80 when the skin melanoma mortality declined in Schleswig‐Holstein. RESULTS The observed mortality decline in Schleswig‐Holstein 5 years after the pilot study was accompanied by a considerable increase in the number of deaths due to malignant neoplasms of ill‐defined, secondary, and unspecified sites (ICD‐10 code C76‐C80) that is not explainable by an increase in the incidence of these neoplasms. Incorrect assignment of 8 to 35 and 12 to 23 skin melanoma deaths per year among men and women, respectively, as ICD‐10 code C76‐C80 during 2007 through 2010 could explain the transient skin melanoma mortality decline observed in Schleswig‐Holstein. Five years after implementation of the program, the nationwide skin melanoma mortality increased (age‐standardized rate change of +0.4 per 100,000 person‐years [95% confidence interval, 0.2‐0.6] in men and +0.1 per 100,000 person‐years [95% confidence interval, ‐0.1 to 0.2] in women). CONCLUSIONS Although the current analyses raise doubts that the skin cancer screening program in Germany can reduce the skin cancer mortality rate, the authors do not believe the program should be immediately stopped. Further in‐depth evaluations are required. Cancer 2016;122:432–437. © 2015 American Cancer Society.
