Benefits of a Straight Laparoscopic Restorative Proctocolectomy With Ileal Pouch Anal Anastomosis for Ulcerative Colitis

Abstract: An sLRP for UC has the advantage over an open restorative proctocolectomy of better short-term outcomes, and both groups have similar long-term outcomes. This procedure is acceptable for minimally invasive surgery in patients with UC.

“…The differences in length of procedure were less pronounced than in previously reported data. The conversion rate from laparoscopic to open surgery was below 1% which is less than the rates from early laparoscopic reports of RPC . These improvements illustrate the effect of the learning curve and improved patient selection over the last decade.…”

“…Several studies have supported the role of laparoscopy by demonstrating faster recovery and better postoperative outcomes, but other reports did not show any no additional benefi ts conferred by performing the procedure laparoscopically. [25][26][27][28][29] While the usage of ASA score may not be accepted by all, it has withstood the test of time and has demonstrated that it is very predictive of outcomes following colectomy. [30][31][32][33] However, we do acknowledge that it does not take into account the impact of various other comorbidities.…”

“…Although the role of the laparoscopic technique in the treatment of UC remains controversial because of its complex nature and limited short‐term benefits, a subset of patients are thought to be good candidates for laparoscopic approach, as most of the patients with UC are young. Also, many may prefer the laparoscopic approach to the open approach in hope of potential benefits of minimal invasiveness . With advances in technology and increased experience with laparoscopic surgery, the feasibility and safety of LAP‐IPAA for UC has been reported in the elective setting .…”
