Benefits of small volume and small syringe for bone marrow aspirations of mesenchymal stem cells

Abstract: In normal human donors, bone marrow aspiration from 30 patients demonstrated a reduced mesenchymal stem cell number in aspirates obtained using a larger volume syringe (50 ml) as compared with a smaller volume syringe (10 ml).

“…Positioning of the Jamshidi needle in the iliac wing was confirmed by fluoroscopy. BMA was collected in a 60-ml syringe in a series of discrete pulls on the plunger (targeting a collection of 5-10 ml per pull), with repositioning of the needle tip between pulls based on the reported enrichment of progenitor cells by Hernigou et al [13]. The BMA was processed using the ART bone marrow concentration system (Celling Biosciences, Austin, TX, USA) to produce a bone marrow concentrated cell preparation.…”

“…The number of colony-forming unit fibroblasts was determined as an indication of the number of MSCs present in the bone marrow aspirate before processing and in the bone marrowderived cell concentrate. The assay has been described previously [15,16]. For muscle, four biopsies were obtained from the gluteus medius and gluteus maximus at surgery; one muscle biopsy was for evaluation of fatty infiltration; and one biopsy muscle sample was examined by light microscopy after staining with hematoxylin-eosin, periodic acid Schiff, and Masson's trichrome to check fatty degeneration.…”

“…Previously reported clinically effective doses of MSC range from 1500 to 9000 CFU-F/mL depending on the diagnostic and the method of BMC delivery [5,11,20,10,8,21] . This wide range seems to be related to the huge interpersonal variability of MSC endowment [22] and the different protocols utilized for quantification, but it also might be associated to the different processed volumes of BM and BMC ( Table 2). Similarly, another parameter associated to BMC´s healing potential is the number of CFU-F per total nucleated cells which ranges from 25 to 39 CFU-F /106 TNC [10,5,11,21,23] .…”
