Hamartomas, also known as fibroadenolipomas, are rare, benign formations that can develop in various organs, including the breast. They present clinically as a soft, mobile nodular lesions and are generally asymptomatic. They may be discovered incidentally during imaging studies performed for other reasons. Owing to the increasingly widespread use of mammographic screening, the diagnosis of breast hamartomas is on the rise. The masses are associated with specific mammographic and sonographic features that reflect their diverse tissue components. They also appear to present reproducible features on elastography. This article reviews the typical features of breast hamartomas seen on these three imaging modalities.Keywords Breast hamartoma Á Ultrasound Á Breast elastography Sommario L'amartoma, o fibroadenolipoma, è una rara formazione benigna che può insorgere in diversi organi, tra i quali la mammella. Clinicamente si presenta con aspetto nodulare soffice e mobile rispetto ai piani circostanti; generalmente asintomatico, può rappresentare un riscontro occasionale nel corso di esami strumentali, in particolare la diffusione dei programmi di screening mammografico ha comportato un maggiore riscontro di tale lesione. Presenta caratteristiche mammografiche ed ecografiche peculiari, legate alle differenti componenti tissutali che lo costituiscono. Anche l'elastonosografia mostra aspetti riproducibili.Vengono presentati gli aspetti tipici di questa lesione con ecografia, mammografia ed elastosonografia.

KEYWORDSColor Doppler; Breast; Mammary lesions.Abstract Has color Doppler a role in the evaluation of mammary lesions? A schematic review of color Doppler diagnostic criteria, indicating a benign or malignant lesion, are described. Malignant tumors have a tendency to show increased vascularization, but avascular tumors may be found as well as hypervascular benign tumors. Number of vessels, blood-flow velocity, and intra-tumoral vessel resistance assessed by color Doppler ultrasonography don't permit clear differentiation between malignant and benign tumors, but intra-tumoral blood-flow analysis well correlates with aggressiveness and histological grade of the mass, so a preoperative assessment using Color Doppler may give preliminary prognostic informations useful for therapeutic planning. Color Doppler ultrasound may be valuable also in assessing the efficacy of neoadjuvant chemotherapy and in particular of antiangiogenesis treatments. Color Doppler ultrasound has many other useful applications in the evaluation of diseases of the female and male breast, that are reviewed in the paper. Color Doppler however is not conceived as a stand-alone examination, but associated with Bmode ultrasound that is quite effective as a diagnostic tool, often not requiring Doppler informations.Sommario Il color Doppler ha un ruolo nella valutazione delle lesioni mammarie? Viene presentata una rassegna schematica dei criteri diagnostici color Doppler, che indicano la benignità o la malignità di una lesione. I tumori maligni hanno la tendenza a mostrare vascolarizzazione aumentata, ma si possono trovare tumori maligni avascolari, così come tumori benigni ipervascolarizzati. Numero di vasi, velocità del flusso, indici di resistenza non consentono una sicura differenziazione tra tumori maligni e benigni, ma l'analisi del flusso intratumorale si correla bene con l'aggressività ed il grado istologico della neoplasia, quindi una valutazione preoperatoria, utilizzando il Color Doppler, può fornire informazioni prognostiche preliminari utili per la pianificazione terapeutica. * Corresponding author.E-mail address: giabusil@tin.it (P. BACKGROUND:The aim of the International Academy of Cytology Yokohama System for Reporting Breast Fine Needle Aspiration Biopsy Cytopathology is to improve cytology practice. This study assessed cytologic diagnoses made with the system and its efficacy when it was applied by pathologists with different levels of experience. METHODS: In all, 1080 cases of breast fine-needle aspiration biopsy (FNAB) over a period of 16 years were reviewed and reclassified with the system. The category distribution and the diagnostic performance were compared with the original diagnoses. The concordance rates for diagnoses from pathologists with different levels of experience were also determined. RESULTS: The distribution of cytologic diagnoses made with the system was as follows: 11.7% were insufficient, 56.6% were benign, 20.1% were atypical, 6.1% were suspicious for malignancy, and 5.6% were malignant. The rates for the insufficient and atypical categories were lower than the original diagnosis rates (13.1% and 23.8%, respectively). Overall, 120 cases (11.1%) were recategorized. Among those recategorized as benign, suspicious, or malignant with follow-up data, 96.7% were correctly reclassified. A significant improvement in diagnostic performance was found with the system (P < .001). Such improvement was also seen in problematic breast lesions, including fibroepithelial lesions, papillary lesions, and low-grade carcinomas. Pathologists with intermediate experience showed a higher concordance with an expert pathologist in the diagnoses than those with short experience (κ, 0.838 vs 0.634). CONCLUSIONS:The system effectively categorized the diagnoses, and the diagnostic performance of FNAB reporting was improved. The structured reporting also enhanced the reproducibility of reporting by pathologists with intermediate experience and, to some extent, those with short experience.
