Beyond Cervical Length: Association between Postcerclage Transvaginal Ultrasound Parameters and Preterm Birth

Abstract: Objective To assess the value of transvaginal ultrasound parameters after cerclage placement in estimating the risk of spontaneous preterm birth. Study Design This is a retrospective cohort at a single tertiary care center from 2013 to 2016. Women carrying a singleton, nonanomalous fetus with cerclage in situ and at least one postcerclage transvaginal ultrasound from 160/7 to 256/7 weeks' gestation were included. In addition to abstracting maternal demographic and obstetric characteristics, two stu… Show more

“…While there are multiple studies assessing cervical length in patients without a cerclage, few studies have assessed cervical lengths and odds of spontaneous preterm birth in patients with cervical cerclages, particularly at 20-24 weeks gestation. Additionally, studies have examined the effect of the location of the cervical cerclage within the cervix [ 10 , 11 ]. Battarbee et al worked to identify ultrasonographic criteria associated with an increased risk of PTB in patients with a cervical cerclage [ 10 ].…”

“…Most of these differences involved the length and the appearance of the cervix as well as the position of the cerclage. The UCA was not significantly different between the two groups [ 17 ].…”

“…A suggested monitoring protocol with serial TVS CL measurements is presented in Figure 12. There have been descriptions of TVS parameters to predict PTB after placement of a cerclage, such as CL 112 , location of the stitch within the cervix, curvature of the cervical canal and presence of funneling or amniotic debris 113 ; however, further TVS evaluation has not been proven beneficial, as no further therapy has been shown to change outcome 114 .…”
