Biaxial serial excision: A technique to deal with benign skin lesions and scars

Abstract: Background:In the best of hands, the ultimate surgical scar of an otherwise well-performed surgery is unpredictable, and surgical techniques are constantly evolving to prevent or revise large scars. The present series uses the principles of serial excision to reduce the eventual scar length.Materials and Methods:Between January 1991 and December 2010, 30 patients were operated upon. In the first stage, a lesion was reduced in two dimensions with the intent to create a smaller lesion with the long axis favourab… Show more

“…Also, it lacks donor site morbidity, and it is not subjected to the potential complications of tissue expanders. Different modalities have been described to improve conventional serial elliptic excisions . One of these is the cake flap technique, which permits transforming circular lesions into elliptic lesions and achieves a z‐plasty effect that reduces tension when closing the defect.…”

“…p.T468M mutation, that was identified in our patient, is one of the two most recurrent NSML-causing mutations. NSML mutations have a dominant negative effect on the growth factor/ ERK-MAPK pathway in contrast to the activating mutations in NS 3. NSML associated PTPN11 mutations display gain-offunction (GOF) mechanism resulting in prolonged RAS-ERK1/ 2 activation as observed in NS.…”

“…For the underdevelopment of antihelix, we have used a combination technique: Mustardé with sutures, associated with a variation of the usual scoring technique (described for Stenström) that uses the principle of Gibson and Davis, to minimize the trauma and complications.
