Bicondylar Hoffa Fracture: Open Reduction Internal Fixation Using the Swashbuckler Approach

Abstract: Coronal plane fractures of the femoral condyle are infrequent injuries and are often missed. Unilateral bicondylar coronal plane fractures are even rarer, with only eight reported cases in scientific literature, and are often associated with other injuries. We present here a case of unilateral bicondylar Hoffa fracture that presented in our emergency department and was managed with open reduction and internal fixation by lateral parapatellar arthrotomy using the swashbuckler approach with satisfactory results.

“…Bicondylar Hoffa fracture involving both the femoral condyles is a rare injury and has anecdotally been reported in the literature [Table 1]. 234567 Most reported cases of bicondylar Hoffa fracture have two separate fracture lines and the two condyles are separated from each other. We describe a rare case of conjoint bicondylar Hoffa fracture where both the femoral condyles were joined by a bridge of intact bone adjacent to the intercondylar notch.…”

“…Mounasamy et al ,9 used the transverse approach because of associated patellar fracture and traumatic transverse arthrotomy in his case. Anterior midline approach,4 12 and modified anterior7 11 (swashbuckler) approach are now the standard operating procedure for closed bicondylar coronal plane fracture. In our case, we utilised the anterior midline approach with medial parapatellar arthrotomy as patella was laterally dislocated and found to be irreducible even under anaesthesia.…”

“…But posteroanterior directed screws is difficult to insert in the clinical scenario. Most of the authors used anteroposterior lag screw 4 5 7 8 12. Controversy exists about the number and diameter of the screws used while most authors used 4–5 large-fragment PTCS 3 4 11.…”

“…We approached the fracture through a swashbuckler technique, Wxed the lateral femoral HoVa fracture, and used the intercondylar fracture gap to repair the tibial avulsion of PCL [6]. We are not aware of any reports to approach the bony avulsion of the PCL and repair through the intercondylar fracture interval.…”
