Bilateral arthrography of the wrist

Abstract: In order to determine the true significance of a positive arthrogram in the investigation of wrist pain, a prospective trial was carried out in 60 patients, using the opposite asymptomatic wrist as a control. Of the 46 patients with positive findings in the symptomatic wrist, 34 (74%), had positive arthrograms in the opposite, asymptomatic, wrist. As a result of this, we conclude that a unilateral arthrogram is of little diagnostic value and we recommend the use of the opposite, asymptomatic, wrist as a contro… Show more

“…Even in patients aged 30 years or less, in whom ICC is often considered pathological [27,31], ICCs were present in two-thirds of symptomatic and one-third of asymptomatic wrists; multiple ICCs also occurred in this age group. A previous study of bilateral single-compartment wrist arthrography in patients with unilateral wrist pain found ICCs in 74% of asymptomatic wrists [32], although the mean age may have been higher than in our series and the exact site of the ICCs was not specified.…”

“…Arthrography can, undoubtedly, demonstrate leakage of contrast between the various intercarpal joints. Herbert et al (1990), however, showed that an arthrogram is of little diagnostic value, unless it can be compared with that of the opposite, undamaged wrist [ 19 ].…”
