Bilateral Hydronephrosis From Retroperitoneal Fibrosis

Abstract: Retroperitoneal fibrosis (RPF) is a rare condition characterized by fibroinflammatory tissue infiltrating and compressing retroperitoneal structures. While mostly idiopathic (idiopathic retroperitoneal fibrosis or IRF), RPF is frequently associated with certain drugs, infections, and malignancies. It is thought to be immunemediated because of response to steroids and RPF is commonly seen with other autoimmune diseases, especially IgG4-related disease (IgG4-RD). IRF is also a part of the chronic aortitis syndro… Show more

“…1,2,5,6,15) Surgical biopsy is a highly invasive procedure, however, except in laparotomy cases. 21,22) Although CT-guided biopsy is considered useful, there is still a risk of complications and the need for multiple punctures. 22) In conclusion, we report a case of suspected IgG4-related periaortitis based on CT findings and subsequent IgG4 measurements.…”

“…Given the absence of radiographic evidence showing dilation of the collecting system, clinicians are posed with a diagnostic challenge in these scenarios. 1 This maybe observed in pelvic malignancies, metastatic breast cancer, non-Hodgkin's lymphoma, sarcoidosis, infections like tuberculosis, and other conditions resulting in chronic inflammation, fibrosis, and lymphadenopathy. [2][3][4] Radiation treatment for testicular seminoma, colon and pancreatic cancer can rarely lead to secondary to retroperitoneal fibrosis.…”

“…Successfully identifying non-dilated obstructive nephropathy demands a heightened suspicion level and collaborative efforts among multiple specialties, including nephrology, urology, radiology, and oncology, to achieve early diagnosis and intervention. 1 A delay in recognizing this potentially reversible cause of acute kidney injury can ultimately lead to adverse events and poor patient outcomes.…”
