Bilateral internal thoracic artery grafting improves long-term survival in patients with reduced ejection fraction: A propensity-matched study with 30-year follow-up

Abstract: Broadly applied BITA compared with SITA grafting in propensity-matched patients provides enhanced long-term survival with no increase in operative mortality or morbidity for patients with normal and reduced EF. The expanded use of BITA grafting should be seriously considered.

“…21 Galbut et al reported that, compared with SIMA grafting, BIMA grafting significantly improved late survival of patients with EF ranging from 0.3 to 0.5. 22 The findings from those studies suggest that the use of BIMA grafting may expand in the future. At present, there is no clear consensus among coronary surgeons whether BIMA grafting should be performed in elderly patients.…”

“…Efficacy of BIMA grafting has been recently reported in patient populations for which it was thought previously to be contraindicated. 20- 22 Kinoshita et al reported that BIMA grafting decreased both all-cause mortality and cardiac-related mortality rates without worsening early outcomes in patients with an estimated glomerular filtration rate <60 ml/min/1.73 m 2 . 21 Galbut et al reported that, compared with SIMA grafting, BIMA grafting significantly improved late survival of patients with EF ranging from 0.3 to 0.5.…”

“…Even today, the CABG remains an excellent therapeutic option for treatment of obstructive coronary artery disease, even in diabetic patients [53,54], in elderly patients [55,56] and in patients with low left ventricular ejection fraction [57]. Alternative procedures, such as using laser beams [58], stem cells [59] and even variations in the use of internal thoracic artery [60] have been described, but are part of the treatment of a special group of patients, which is certainly not fit into the daily routine.…”

“…Common vascular conduits used in CABG are internal thoracic artery, saphenous vein, radial artery, right gastroepiploic artery, ulnar artery. And such wide variety of options has caused controversy and no consensus regarding superiority of graft has been fully established [1][2][3].…”
