Bilateral premacular sub‐hyaloid hemorrhage‐unmasking COVID‐19 induced pancytopenia

Abstract: Fundus photo of right and left eye of the patient showing large "boat-shaped" haemorrhages (blue arrows) in pre-macular area of both eyes typical feature of the sub-hyaloid variety with few superficial haemorrhages in posterior pole of both eyes with no evidence of neovascularization or vasculitis

“…This dermatological manifestation has been reported in another case of COVID-induced panctyopaenia; 21 however, although a rare presenting symptom, spontaneous bleeding in an otherwise healthy individual can also be the first presentation. 20 Pancytopaenia can create dilemmas in the clinical management of COVID-19 particularly around interventions with increased bleeding risk, such as venous thromboembolism prophylaxis and performing invasive procedures such as arterial line and central venous cannulation where covered with blood products is likely to be required. As described in the literature, and this case report, management of COVID-19-induced pancytopaenia is primarily supportive.…”

“…Our report of de novo COVID-induced pancytopaenia is not unique. Several cases have been reported throughout the pandemic of de novo pancytopaenia in adults with no haematological conditions or risk factors for cytopaenia 17–20. Two cases of spontaneous bleeding (premacular subhyaloid haemorrhage20 and spontaneous haemoperitoneum21) in healthy male patients with no comorbidities have been reported as presenting symptoms of COVID-19-induced pancytopaenia.…”

“…[ 20 ] While bilateral simultaneous pre-foveal hemorrhage as a complication of post COVID-19 sepsis has not been reported, bilateral premacular sub-hyaloid hemorrhage unmasking COVID-19 infection has been reported. [ 21 ]…”

“…Generally, those affected with COVID-19 manifest with respiratory pathology, especially the classic bilateral multifocal pneumonia; less commonly, however, the infection affects the bone marrow, hindering the hematopoietic system. 1 This may lead to lymphopenia, neutropenia, and rarely pancytopenia. 2 , 3 The first case of persistent pancytopenia associated with SARS-CoV-2 bone marrow infiltration in an immunocompromised patient was described in October 2020, and subsequently other reports emerged.…”
