Bile Leak From Duct of Luschka After Liver Transplantation

Abstract: Excessive use of electrocautery to the liver bed during donor cholecystectomy may injure subcapsular ducts in the gallbladder fossa. In liver transplantation, dissection should be kept close to the serosal lining of the gall bladder, preserving the areolar tissue in the gall bladder bed, to avoid injury to the duct of Luschka.

“…In addition, injuries to these ducts have occurred more frequently in the laparoscopic era, as have other bile duct injuries. Leakage from an injured duct of Luschka has been reported after orthotopic liver transplantation [8]. Primary papillary adenocarcinoma of the subvesical duct has also been described in the literature [9].…”

“…The long-term prognosis is relatively worse. 7 , 9 Less frequent causes include leaks from the cut surface of a liver graft that has been surgically reduced or obtained in a split-liver or living-donor procedure, 10 leaks from unobserved accessory ducts in the gallbladder fossa, 22 and leaks at incompletely closed cystic duct remnants. 23…”

“…It is a common bile associated biliary leakage and intrahepatic biloma formation. 44,45 Biliary ischemia can result from acute or delayed thrombosis of the hepatic artery or postoperative stenosis between the donor and the recipient hepatic arteries. 45 Most liver transplants in the United States are performed using cadaveric donors, but transplantation with living related donors are also performed.…”
