Biliary cystadenoma with mesenchymal stroma: Report of a case and review of the literature

Abstract: Biliary cystadenomas are rare, cystic neoplasms of the biliary ductal system that usually occur in middle-aged women. They cannot be safely differentiated from cystadenocarcinomas before operation and should always be considered for resection. Cystadenomas have a strong tendency to recur, particularly following incomplete excision, and a potential of malignant transformation. Therefore, complete resection is the therapy of choice and thorough histopathologic evaluation is imperative. A case of benign biliary c… Show more

“…Hepatobiliary cystadenomas are rare, benign cystic lesions which usually arise in the liver (80-85%) in the extrahepatic bile ducts and rarely in the gallbladder [3,4]. Approximately 85-95% of the patients are women typically occurring in the fifth decade of life [5].…”

“…The differential diagnoses include simple cysts, parasitic cysts, post-traumatic cysts, cystic degeneration of liver neoplasm [4,6]. The most challenging one is hemorrhagic cysts where US may mistaken the internal clots for papillary nodules and irregular septa.…”

“…Although biliary cystadenocarcinoma is rare, liver cysts have high prevalence being present in 2.5% of the population [119] and cannot be distinguished from cystic biliary tumours on the basis of CA19-9 and CEA levels [14,114]. However, core biopsy is unlikely to yield sufficient tissue in case of simple cyst or cystadenoma; it also is not suitable for the diagnostics of focal malignancy and rarely can lead to peritoneal carcinomatosis [13]. Therefore radiological diagnostics, especially computed tomography, is essential [117].…”

“…We have previously described a case of huge biliary cystadenoma in a middle aged woman and presented an extensive review of this uncommon entity [1] . We report, from the same institution, a case of synchronous, multiple, huge biliary mucinous cystadenomas with unique features.…”
