Biliopancreatic Diversion with Transitory Gastric Restriction and Duodenal Bulb Preservation: 88 Patients since 1992

Abstract: BPD-TGR appears to be an effective operation with few complications and also a satisfactory correction for failed gastric restrictive procedures, or even a sequential operation in the super-obese.

“…This procedure reduces the likelihood of dilatation in time (which could possibly be responsible for the partial failures observed in the long-term with the sleeve resection), involves no radical intervention, and allows complete anatomical reversibility. 10,11 In conclusion, the procedure adopted in this study, involving the prolongation of the Magenstrasse to 3-4 cm proximal to the pylorus combined with pyloroplasty, confers to this restrictive intervention some characteristics similar to gastric bypass, i.e. rapid transit of the alimentary bolus in the mill and a scarse reflux in the gastric fundus, with possible effects on the entero-endocrine system and consequent loss of interest in food.…”
