Billroth 1 versus Roux-en-Y reconstructions: a quality-of-life survey at 5 years

Abstract: The results from this study show that, at 5 years, both symptomatically and functionally, Roux-en-Y reconstruction was superior to the Billroth I method after subtotal distal gastrectomy for gastric cancer. However, the overall outcome in both groups was good, with patient satisfaction scores of around 75% in each group.

“…Patients infrequently experienced heartburn (6.7%) and only two patients had endoscopic evidence of reflux esophagitis. On the other hand, bile reflux into the remnant stomach was observed endoscopically in 73.5% of our patients, a rate higher than what has previously been reported [8,10,11]. However, the incidence of remnant gastritis was low and severe symptoms were rarely observed.…”

“…Miyagaki et al [12] reported the reduction (13.5 %) after distal gastrectomy in their study was larger than in other studies (range, 9.1-10.5 %). In our study, the reduction after distal gastrectomy was 11.4 % on average (data not shown), which was also slightly larger than in other studies [13,14]. Weight loss is considered a risk factor for internal hernia in RYGBP [15,16].…”

“…Increased F-18 FDG uptake in inflammatory and infectious lesions is well established [23,27,28]. It is well documented that patients with a gastroduodenostomy have reflux of duodenal contents into the remnant stomach due to the absence of the pyloric sphincter, leading to gastritis [14,17]. Therefore, several factors such as normal physiologic uptake, inflammation and fibrosis after surgery could contribute to the persistent increase in F-18 FDG uptake at the anastomotic site.…”

“…Its utility has been well demonstrated in many studies, especially in cases of intraluminal recurrence [13][14][15][16][17]. However, there are some limitations to endoscopy.…”
