Biliothorax is the presence of bile in the pleural cavity. This condition is rare, and it usually results as a complication of hepatobiliary procedures. The authors present a case of an 87-year-old female who was admitted to the emergency department with the acute onset of severe dyspnea. A chest X-ray and CT revealed a large right-lung pleural effusion that, after thoracentesis, confirmed the presence of biliothorax. It is important to consider this entity when confronted with an effusion liquid of a dark greenish color, as a delay in diagnosis and management may be life-threatening.

Introducción: El biliotórax es una condición infrecuente definida por la presencia de bilis en el espacio pleural. Actualmente, hay alrededor de 70 casos descritos en la literatura. Sigue siendo relativamente desconocido, por lo tanto, poco sospechado. Esta entidad suele ser el resultado de una lesión iatrogénica, a menudo secundaria a cirugías o traumatismos del tracto biliar, que conduce a la formación de una fístula pleurobiliar.Caso clínico: Presentamos el caso de un paciente con probable cáncer pulmonar metastásico a pleura e hígado quien por la enfermedad hepática extensa desarrolló biliotórax como complicación.Conclusión: Este caso resalta la importancia de tener un alto grado de sospecha de biliotórax en pacientes que presentan derrame pleural causado por cáncer de pulmón con enfermedad hepática metastásica extensa. Si bien es un diagnóstico raro, el biliotórax puede provocar complicaciones respiratorias y morbilidad significativas.

Brown‐colored pleural effusion is rare and may result from high bilirubin levels such as bilothorax (often described as a pleural fluid‐to‐serum bilirubin ratio of > 1.0). We describe four patients with malignant pleural effusion that appeared macroscopically brown with a pleural fluid‐to‐serum bilirubin ratio between 3.7 and 16.2. All had metastatic adenocarcinomas; three were from lung and one from gastric origin. None demonstrated clear pleurobiliary fistulas on investigations. Postulates for the development of brown effusion include heme oxygenase 1 overexpression in malignant cells situated in the pleura, intrapleural hemolysis, passive movement of bile through microscopic diaphragmatic pores, and drainage of biliary fluid into the pleural lymphatics.
