Biomarkers of Acute Kidney Injury

Abstract: Acute kidney injury (AKI) is a common condition with a high risk of death. The standard metrics used to define and monitor the progression of AKI, such as serum creatinine and blood urea nitrogen levels, are insensitive, nonspecific, and change significantly only after significant kidney injury and then with a substantial time delay. This delay in diagnosis not only prevents timely patient management decisions, including administration of putative therapeutic agents, but also significantly affects the preclini… Show more

“…We believe our description of Roundup-induced apoptosis is novel.. KIM-1 is a type I cell membrane glycoprotein expressed by tubule epithelial cells in response to injury. It appears in urine after proximal tubular injury (Ferguson et al, 2008;Hoffmann et al, 2010;Vaidya et al, 2008a). In this study, uKIM-1 outperformed other plasma and urinary biomarkers in early detection of AKI.…”

“…Cys-C is a 13-kDa protein that is freely filtered via the glomerulus but reabsorbed and degraded in lysosomes of proximal tubule cells. It is not secreted by kidney tubules (Ferguson et al, 2008;Vaidya et al, 2008a). Recently, serum or pCys-C levels were found to perform better than pCr in some patients as a surrogate measure of kidney function (GFR) .…”

“…Several promising biomarkers have emerged from these studies (16,17). Analysis of temporal and spatial differences in their expression highlights the cellular complexity of the AKI response (ref.…”

“…In patients with preexisting renal impairment, a nephrologist should assess the risk of postprocedural dialysis. In all patients, we advocate careful technique, minimal contrast load, and adequate postprocedural hydration [9,18]. Despite these precautions, catastrophic renal failure risk may remain, as the precise pathophysiology of contrastinduced nephropathy has yet to be identified [10,15,17].…”
