Biopsy needles for mediastinal lymph node sampling by endosonography: current knowledge and future perspectives

Abstract: Due to the increasing role of endosonography [endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and esophageal ultrasound-guided fine needle aspiration (EUS-FNA)] in the diagnosis of several lung diseases, the knowledge of technical aspects is mandatory to optimize the success of the procedure. Among those technicalities related to the procedure, the choice of a needle over another one-either in terms of dimension and type-may have a role in the diagnostic process, especially in some… Show more

“…The experience of the two patients indicates that the application of CEUS in re-biopsy is potentially a better choice when compared to the use of larger core needle for those with an unsuccessful diagnosis at the first puncture. Additionally, several previous studies suggest that larger needle size plays a slightly role for CNB of lymphoma (21)(22)(23)(24).…”

“…Endobronchial ultrasound (EBUS) is a technique recently developed by thoracic endoscopists for real-time ultrasound-guided needle biopsy of mediastinal lymph nodes for staging lung cancer [3]. It is carried out using a modified bronchoscope having a small ultrasound convex probe at the tip allowing to perform ultrasonography examination of the mediastinum by applying gentle pressure to the trachea or the bronchial inner surface (Fig.…”

“…However, all of those factors have been previously evaluated in a meta-analysis and no such association was demonstrated. 10 Moreover, our standard process of ≥ 50 needle strokes per target might seem controversial to the reader. The BTS 2011 Guideline for Advanced Bronchoscopy does not mention an optimum number of needle passes or strokes.…”
