Birth-related spinal cord injuries: a multicentric review of nine cases

Abstract: The pregnancy was uneventful in all cases. The fetal presentation was cephalic in three cases, a breech presentation in four cases, a face presentation in two cases. The lesion involved the cervical spine in six cases. Three patients presenting upper cervical injuries died before the age of 6 years. The six remaining patients experienced no neurological improvement. These rare conditions occur during difficult deliveries with abnormal presentations, the most common being a breech presentation with entrapment o… Show more

“…Breech presentation complicated by entrapped fetal head has been found to be responsible for many reported cases [ 18 ]. Vertebral fractures or spinal dislocations can be associated [ 18 ]. A lateral radiograph of the spine should be obtained to demonstrate vertebral fracture/subluxation.…”

“…Neonatal spinal cord injury resulted from birth trauma has been noted since the nineteenth century (1). The exact mechanism of birth related spinal cord injury is unclear, but is believed to be caused by excessive extraction, rotation, or hyperextension of the baby's neck during delivery such as shoulder dystocia, breech vaginal delivery, and difficult deliveries with forceps or vacuum assistance (2)(3)(4)(5). The upper and middle cervical spine are injured mostly in vertex delivery, and the lower cervical and upper thoracic region are involved majorly in breech delivery (3).…”

“…Additionally, the model can also be useful for better understanding the pathological processes related to child SCI, a scarcely researched field. In fact, clinical studies report a relatively high occurrence of SCI among very young children (Bilston and Brown, 2007;Pieretti-Vanmarcke et al, 2009), even at perinatal age (Vialle et al, 2008).…”
