Blood transfusion is associated with increased resource utilisation, morbidity and mortality in cardiac surgery

Abstract: The purpose of the present investigation was to examine the impact of blood transfusion on resource utilisation, morbidity and mortality in patients undergoing coronary artery bypass graft (CABG) surgery at a major university hospital. The resources we examined are time to extubation, intensive care unit length of stay (ICULOS) and postoperative length of stay (PLOS). We further examined the impact of number of units of packed red blood cells (PRBCs) transfused during PLOS. This is a retrospective observationa… Show more

“…Several studies have shown that any amount greater than 1 unit of PRBC transfused, regardless of the procedure performed, results in decreased life expectancy of the patient [9][10][11]. In contrast to our study, other publications compared standard CPB with the system in which low crystalloid filling was used, and the postoperative day 1 drain output was comparable in both groups [12]. However, in our study the 'mini-CPB' group had a total drain output decrease of 30%.…”

“…Several other studies have also found similar effects of blood transfusions and inotropic support on mechanical ventilation duration. [18][19][20] Kogan et al 21 found that kidney failure, stroke, peri-op bleeding and infection as main contributing factors of delayed extubation after surgery. Higgins et also found kidney failure as a risk factor of delayed extubation.…”

“…Blood transfusion is one of the most important problems after CABG; patients who had received blood had longer extubation time, ICU and hospital stay, higher mortality and morbidity, and operational complications (22,23). Previous studies have shown that anaemia is more prevalent than other surgeries in cardiac surgery (24).…”
