Blunt and penetrating chest injuries

Abstract: The management of chest injuries begins with knowledge of what happened to the patient at the time of the traumatic incident and converting this information into possible diagnoses. The various organs of the chest cavity are discussed emphasizing the controversies that attend the management or diagnosis of each one.

“…Interestingly, this acute pain is attenuated by resetting and stabilizing the mineralized bone/apposed periosteum (Fig. 4) at the site of fracture and this intense pain can be reactivated within seconds by movement and mechanical distortion of the fractured femur (McSwain, 1992; Hedequist et al, 1999; Santy and Mackintosh, 2001; Bone et al, 2004; Camuso, 2006). This clinical picture would suggest that mechanosensitive nerve fibers including those present in the densely innervated periosteum, are pivotally involved in driving the initial fracture pain.…”

“…Da 75 % der Unfallpatienten zum Zeitpunkt der Erstuntersuchung keine äußeren Verletzungszeichen am Brustkorb aufweisen, kommt der Analyse des Unfallmechanismus eine besondere Bedeutung zu [14]. Sie erlaubt Rückschlüsse auf Art und Ausmaß des Thoraxtraumas.…”

“…Several precordial impactions with cardiac squeezing between the sternum and spine is one possible mechanism [3,4]. In the English literature, however, this is the first case of a patient with pectus excavatum having a blunt rupture.…”

“…Patients with cardiac rupture following blunt thoracic trauma rarely survive and most die at the scene of the accident or soon after in the emergency room, before the cardiac lesions are detected [1,2]. Cardiac squeezing between the sternum and spine is postulated as the most likely trauma mechanism [3,4]. …”
