Bone graft substitutes in anterior cervical discectomy and fusion

Abstract: Anterior cervical discectomy with fusion is a common surgical procedure for patients suffering pain and/ or neurological deficits and unresponsive to conservative management. For decades, autologous bone grafted from the iliac crest has been used as a substrate for cervical arthrodesis. However patient dissatisfaction with donor site morbidity has led to the search for alternative techniques. We present a literature review examining the progress of available grafting options as assessed in human clinical trial… Show more

“…The iliac crest has long been recognized among many spine surgeons as a reliable source for cancellous bone. 1,8,13 However, the iliac crest is not an ideal donor site, because a variety of donorsite morbidities can result from its use. 2,7,12,[15][16][17] This has led to the search for other autologous donor sites.…”

“…Adding fusion to the ACD operation diminishes SH losses and in parallel prevents foraminal compressions (18,21) . Although autografts provide better fusion rates, because of donor site complications and the fact that it requires more time, cage and artificial grafts are preferred (7,17,20) . Prevention of postoperative kyphosis is another advantage of cage fusion.…”
