Bone metastases as the initial presentation of carcinoma of gall bladder: a rarity

Abstract: Distant metastases are rare form of presentation of carcinoma gall bladder. Bony pain as initial presentation is quite unusual. A 50-year-old woman presented with the pain in right shoulder. Investigation showed metastatic adenocarcinoma in the head of humerus and the primary was found in the gall bladder. “…[3] Metastases frequently involve liver (76-86%) and regional lymph nodes (60%). [4] Distant metastasis to virtually all organs is possible[345] and has been reported but distant metastasis to bone is rare. [456] The reason may lie in the rapidly growing nature of this malignancy that causes the demise of the patient early due to local infiltration, leaving little time for the development of bony metastasis.…”

“…To the best of our knowledge, less than 10 cases have been reported in literature with gall bladder malignancy, clinically presenting with clinical features of skeletal secondaries. The reported sites of skeletal secondaries were femur, clavicle, humerus, calvaria, and lumbar vertebrae [31][32][33][34]. The recognition of skeletal metastasis is clinically important to tailor the appropriate palliative care, even when the bony disease is clinically silent.…”

“…It spreads mainly to the liver and regional lymph nodes along the cystic duct, common hepatic duct and common bile duct and to pre and para-aortic lymph nodes but distant metastasis has also been explained in the literature. 2,3 Here in we report a case of carcinoma of the gallbladder metastasizing to the heart and presenting with features of malignant pericardial effusion. This is 7th such case to report cardiac metastasis from gallbladder carcinoma.…”
