Bordetella bronchiseptica pneumonia in an immunocompetent pig farmer

Abstract: Highlights B. bronchiseptica has rarely been isolated from humans despite exposure to animals. Working with farm animals, COPD or AIDS are risk factors to develop pneumonia. B. bronchiseptica pneumonia may lead to misdiagnosis with other infections.

“…The severity of the infection and the resulting weight loss depends on age and immune status of the pig, the virulence of the strain of B. bronchiseptica , and the presence of other infections or underlying health conditions 4 , 5 , 12 . In general, pigs after weaning tend to be more susceptible to infections and may experience more severe symptoms 15 , 31 , 51 .…”

“…The bacteria spread to the nasal epithelial lining by the airborne channel, leading to chronic infections 3 . Lung infection is a severe disease caused by B. bronchiseptica , particularly in intensive piggery systems , which was the main mortality reason in weanling pigs before introducing vaccines 4 , 5 . Currently, vaccination against B. bronchiseptica is a routine procedure for avoiding illnesses in animals 6 .…”

“…Recently, there have been several reported human cases of B. bronchiseptica infections with risk factors including COVID-19, HIV, lung malignancy and decompensated liver cirrhosis; however, we only found 2 previous case reports of B. bronchiseptica infections with the primary immunocompromising risk factor being related to alcohol use [ 3 , 5 , 9 , 11 , 13 ]. Additionally, a recent case report describes a pig farmer that contracted B. bronchiseptica [ 2 ] . Although our patient was COVID-19 and HIV negative without signs of liver injury, her history of chronic alcoholism and animal exposure could have played a role in the severity of her respiratory infection.…”

“…In contrast, B. bronchiseptica infects a variety of animals, often establishing chronic infections that range from lethal pneumonia to asymptomatic carriage and is capable of surviving in the environment ( Goodnow, 1980 ; Brockmeier et al, 2019 ). While rare and more frequent in immunocompromised individuals, human infections have been reported ( Register et al, 2012 ; Agarwal et al, 2022 ; Barcala Salido et al, 2022 ; Moore et al, 2022 ).…”
