Botulinum toxin: evidence-based medicine criteria in rare indications

Abstract: Presently, there are more than 50 possible indications for the application of botulinum toxin. In some indications the use of botulinum toxin has already been approved; others are about to be approved. For most of these indications a sufficient number of studies have been published. For rare indications, an insufficient number of studies are available. In part there are only published case reports. Classification according to the criteria of evidence-based medicine is thus difficult. Here we try to describe th… Show more

“…For the discipline of pharmacology, the rational approach to the use of drugs, including evidencebased information (in the form of clinical drug trials), the perils of drug misuse, and unbiased sources of drug information are of paramount clinical interest and should be emphasized in any medical school curricular design. The recent emphasis on "evidence-based" approaches in medicine and medical education (Barnett et al 2000;Ghali et al 2000;Welch and Lurie 2000) has strong pharmacological implications (Felker and O'Connor 2001;Jost and Kohl 2001). This approach is emphasized at SIUSM in sessions dealing with the evaluation of clinical drug trials and identification and evaluation of unbiased sources of drug information.…”

“…A recent publication details over 50 uses for botulinum toxin [12]. In spite of lack of FDA approval, many of the uses have demonstrated benefit and safety in clinical trials or patient series including botulinum toxin injection of the cricopharyngeus for dysphagia [13,14].…”

However, it can be difficult clinically to distinguish the recurrent, simple focal motor seizures of EPC from the other hyperkinetic movements, especially myoclonus, occurring in patients with established RS.…”
