“…In our opinion, this is exactly where preoperative chemical components separation using botulinum might play a key role. Lledo et al (37) published in 2020 their experience from a comparative study on 80 patients with midline incisional hernias, in which chemical components separation down-staged the required hernia repair type from components separation to a Rives-Stoppa repair with a 100% primary fascial closure achievement rate in the chemical components group and no complications from the injection of botulinum per se. Interestingly, large midline incisional hernias (hernia W3 according to EHS classification) with defects of 12-18 cm in diameter were only included in the study (21).…”

“…Of these 75 articles, 20 were selected for inclusion. 10e12, 14,21e36 One article was a case-control study, 11 2 articles were non-randomized trials, 34,36 1 was a case series with selected controls, 29 and all other articles were single-arm prospective or retrospective studies. 10,12,14,21e28,30e33,35 Owing to the large number of single-arm studies, a minor deviation from the originally registered protocol 37 took place in the form of the additional use of the MINORS checklist for quality assessment, as this checklist is both applicable for comparative and noncomparative studies.…”

“…is variable according to the characteristics of the incision, location, type, size, preferences of the surgeon, etc. Complex incisional hernias may require abdominal preparation before surgery with infiltration of botulinum toxin and sometimes also with the creation of a progressive pneumoperitoneum [35,36]. Hernia repair is thus characterized by the increasing use of prosthetic meshes made of pure or different hybrid biomaterials, depending on the type of surgery [37].…”
