Botulinum toxin treatment of secretory disorders

Abstract: Botulinum neurotoxin serotype A (BoNT/A) has revolutionised the treatment of a variety of autonomic hypersecretory disorders. Several open and controlled studies indicate that BoNT/A is a safe and effective treatment for focal hyperhidrosis of the axillae and palms, for gustatory sweating, and for some other rare conditions associated with focal hyperhidrosis. There is class I evidence for the efficacy of botulinum toxin in axillary hyperhidrosis and class II evidence for palmar hyperhidrosis and gustatory swe… Show more

“…Botulinum toxin can also be an effective treatment for excess sweating, and may be helpful in individual patients with PD with focal hyperhidrosis. 813,814 DBS-STN has also been reported to ameliorate sweating during "off " periods in fluctuating patients. 815 Severe hyperpyrexia can be seen after levodopa withdrawal, probably as a manifestation of a neuroleptic malignant syndrome, and should be promptly treated by reinstitution of dopaminergic agents.…”

“…BoNT-A also blocks cholinergic parasympathetic and postganglionic sympathetic nerve synapses in the autonomic nervous system. Hence, BoNT-A is also used for treating overactive smooth muscles (anal fissure and outlet-type constipation, esophageal achalasia) as well as for secretory disorders (focal hyperhidrosis, gustatory sweating, sialorrhea) (Naumann and Jost, 2004).…”

“…18 In addition, BoNT/A inhibits acetylcholine release at autonomic nerve terminals that innervate glands and smooth muscles, and it is currently used to treat various forms of hyperhydrosis. 20 At present, three different BoNT/A formulations are available in Europe: Botox (Allergan, Irvine, California), Dysport (Ipsen, Slough, UK), and Xeomin (Merz Pharmaceuticals, Greensborough, North Carolina). BoNT therapy has a remarkable record of safety, and this depends partly on the toxin's ability to remain relatively localized at the site of injection.…”
