Bowel perforation after paediatric orthotopic liver transplantation

Abstract: Bowel perforation is a well-recognized complication following orthotopic liver transplantation. Of 194 paediatric liver transplantations performed in our hospital, 13 patients (6.7%) developed bowel perforation post-transplantation. Contributory factors included previous operation, steroid therapy and viral infection. The incidence was higher in children who underwent transplantation for biliary atresia after a previous Kasai portoenterostomy. Seven patients (53% of this group) reperforated. Diagnosis may be d… Show more

“…Current literature is scant on intestinal perforations, and most are anecdotal reports. We showed here that the incidence of intestinal perforation in adult liver transplant recipients was 2.7%, which is consistent with previous studies reporting an incidence of 1% to 5.3% for adult intestinal perforations after orthotopic liver transplantation 36. The reported incidence in this study is similar to that by Xiong et al1 who studied 187 Chinese adult liver transplant recipients and detected an incidence of 3.2%.…”

“…Intestinal perforation is reported to occur in 4%–20% of children undergoing LT for biliary atresia124, 125 and should be suspected in cases of unexplained sepsis or clinical deterioration. Treatment for presumed fungal infection should also be strongly considered in these cases 126.…”
