Brain Apparent Diffusion Coefficient Evaluation in Pediatric Patients With Neurofibromatosis Type 1

Abstract: The higher ADC values in children with NF1 suggest an increase in water content of the normal-appearing brain. The UBOs are the areas with the highest water content. The regressed UBOs sites show higher water content than the normal-appearing areas.

“…These findings confirm those of previous reports. [6][7][8][9]14 However, the ADC values found in this study are not as high as the ADC values found previously. An explanation of lower ADC values may be found in the shorter effective TE and the different b-value that were used in our scanning protocol compared with reported DWI protocols.…”

“…On the basis of the high ADC values, a widespread myelin disorder was suggested to be present in patients with NF-1. [6][7][8][9] However, ADC reflects only the magnitude of the diffusion of water molecules. Although high ADC values might suggest increased water content of the brain, with ADC alone, it is not possible to examine the microstructural integrity of the parenchyma.…”

“…However, all the WM NAS parameters (ADC, FA, AD and RD) were decreased compared to controls which appears to be a counterintuitive finding. Previous studies reported higher ADC values in normal-appearing brain regions in NF1 patients compared to controls 11,14 . Additionally, FA values in the thalami have been reported to be lower in NBOs compared to those in controls 18 .…”

“…The most common CNS lesions in the brain of NF1 patients are the so-called NBOs 5,6,14 . Histopathologically these lesions represent areas of vacuolar/ spongiotic changes with increased fluid within the myelin 9 .…”
