The central nervous system (CNS) and kidneys are tightly interconnected in an organ cross talk, "Brain-Kidney" connection. 2. The incidence of AKI in the intensive care unit (ICU) involves up to 50% of ICU admissions. Many of those patients require some type of renal replacement therapy (RRT). 3. Critical patients with acute brain injury (ABI) are part of a population of critical patients where the prescription of RRT must consider the risk of secondary brain injury and address main CNS physiological variables (CBF, CPP, and ICP). 4. During RRT, spectral Doppler waveform evaluation (PSV, EDV, and MFV) and the pulsatility index (PI) that is derived from it are very useful to estimate "in real time" possible modifications in regional cerebral blood flow. 5. The dialysis disequilibrium syndrome (DDS) can occur in any patient under RRT, but it is observed more frequently when patients are exposed to their first RRT session, when control of the rate of urea removal is crucial.

Санкт-Петербургский государственный университет, Российская Федерация, 199034, Санкт-Петербург, Университетская наб., 7-9 Рассмотрены клинические и патогенетические аспекты изменения работы внутренних органов, в частности сердца и почек, в результате острого нарушения мозгового кровообращения. Развитие цереброкардиального синдрома характеризуется возникновением в ранее интактном миокарде на фоне дисбаланса вегетативной нервной системы и катехоламиновой цитотоксичности ишемических изменений, нарушений ритма сердца. Изменение функционального состояния почек связывается с активацией провоспалительных цитокинов, повышением симпатической активности и нарушением работы гипоталамо-гипофизарной системы, происходящих в рамках взаимодействия нервной, эндокринной, аутакоидной систем. Предполагается ведущая роль микроангиопатии как системного процесса, развивающегося при метаболическом синдроме, в формировании полиорганной патологии с поражением головного мозга, почек и сердца. Выявление микроангиопатии в одном органе у таких пациентов может быть предиктором острого нарушения мозгового кровообращения и других сосудистых расстройств. Библиогр. 44 назв. Табл. 1.Ключевые слова: инсульт, цереброкардиальный синдром, почечная недостаточность, микроангиопатия, метаболический синдром.This review examines clinical and pathophysiological aspects of internal organ diseases; in particular, post-stroke heart and renal disturbances are discussed. Progression of cerebral-cardiac syndrome can be described as the emergence of ischemic and arrhythmic changes because of autonomic imbalance and catecholamine cytotoxicity in previously intact myocardium. Change in renal functional state can be explained with activation of proinflammatory cytokines, the sympathetic nervous system, and also hypothalamic-pituitary disturbances through the interaction of the nervous, endocrine, and autacoid system. The article argues for the principal role of microangiopathy as a systemic process, taking place in metabolic syndrome and resulting in polyorganic insufficiency with major dysfunction of the brain, heart and kidney. Diagnostics of microangiopathy in one of these organs can predict stroke and other vascular diseases. Refs 44. Table 1.

Background & Objective: Traumatic brain injury is an outcome of the physical or mechanical impact of external forces on the brain. Thus, the silent epidemic has complex pathophysiology affecting the brain along with extracranial or systemic complications in more than one organ system, including the heart, lungs, liver, kidney, gastrointestinal and endocrine system. which is referred to as Multi-Organ Dysfunction Syndrome. It is driven by three interconnected mechanisms such as systemic hyperinflammation, paroxysmal sympathetic hyperactivity, and immunosuppression-induced sepsis. These multifaceted pathologies accelerate the risk of mortality in clinical settings by interfering with the functions of distant organs through hypertension, cardiac arrhythmias, acute lung injury, neurogenic pulmonary edema, reduced gastrointestinal motility, Cushing ulcers, acute liver failure, acute kidney injury, coagulopathy, endocrine dysfunction, and many other impairments. The pharmaceutical treatment approach for this is highly specific in its mode of action and linked to a variety of side effects, including hallucinations, seizures, anaphylaxis, teeth, bone staining, etc. Therefore, alternative natural medicine treatments are widely accepted due to their broad complementary or synergistic effects on the physiological system with minor side effects. Conclusion: This review is a compilation of the possible mechanisms behind the occurrence of multiorgan dysfunction and reported medicinal plants with organoprotective activity that have not been yet explored against traumatic brain injury and thereby, highlighting the marked possibilities of their effectiveness in the management of multiorgan dysfunction. As a result, we attempted to respond to the hypothesis against the usage of medicinal plants to treat neurodegenerative diseases.
