Background: Around 200,000 breast disorders are annually diagnosed all over the world. Fibrocystic changes are the most common breast disorder and fibroadenoma is the most prevalent benign breast tumor. The present study aimed to determine the spectrum, type and prevalence of breast masses in women referred to Shiraz University of Medical Sciences between 2004 and 2012 . Materials and Methods: A cross-sectional study was conducted on the diagnostic reports data. Results: A total of 640 samples were studied. Most 57.3% of masses were detected in the left breast, 65%, 28.2% and 6.1% of cases presenting with benign, malignant, and inflammatory lesions, respectively. Among all the samples the most prevalent diagnosis (37.7%) was fibroadenoma and fibrocystic lesions (17%). 174 samples (96% of the malignant cases) were invasive. 6.5% of the benign, and 37% of the malignant cases occurred in post menopause women and the differences were statistically significant. Among those with malignant tumors lymph nodes were involved in 25.6% of menopausal women and 44.2% of non-menopausal ones, and the difference was statistically significant. Conclusions: Regular clinical breast examination beside mammographic follow-ups, especially during menopause, should be carried out as a priority and a national organized program should be designed for screening breast disorders.

ResumenSe presenta el caso de una mujer de 20 años de edad con crecimiento mamario severo bilateral tipo gigantomastia, de tres meses de evolución, quien fue valorada inicialmente en hospital de primer nivel de complejidad, donde se le hizo diagnóstico de mastitis aguda, la cual no tuvo respuesta a los antibióticos y cuya biopsia de mama fue sospechosa para angiosarcoma mamario, por lo cual remiten a un tercer nivel. Allí fue valorada por el grupo de mastología que reorienta el diagnóstico luego de exámenes complementarios, documentándose así una poco frecuente asociación entre hipertrofia virginal mamaria e hiperplasia pseudoangiomatosa del estroma mamario difusa. Se revisa la presentación clínica de ambas entidades y su diagnóstico diferencial.Palabras clave: Hiperplasia, Hipertrofia, Mesodermo, Mama. AbstractPresents a woman of 20 years old with severe bilateral mammary growth rate type gigantomastia with three months of duration. Initially valued and diagnosed at first level as an acute mastitis, which didn't respond to antibiotics. A mammary biopsy was suspicious for breast angiosarcoma, and refered, valued by mastology that reorients the diagnosis after further examination and biopsy, and documenting a rare association between virginal breast hypertrophy and difuse pseudoangiomatous hyperplasia of the mammary stroma, which together with a good diagnosis could receive appropriate treatment. The clinical presentation of the diseases and their differential diagnosis is reviewed.

A Long Term Review of Surgically Treated Enlarged Breasts

Introduction: Breast hypertrophy (excessive enlargement of breast), a condition though rare, is well pronounced in some females. Surgery either by reduction mammaplasty or sub-cutaneous mastectomy is often employed in treatment of this condition. Aim: This study seeks to find out the long term level of satisfaction of the surgery on these persons after about 10 -12 years after treatment. Materials and Methods: From January 1994 to December 2000, the Komfo Anokye Teaching Hospital (KATH) carried out surgeries on 43 females who presented with several degrees of breast hypertrophy. Patients were asked to answer questionnaires on their condition pre-operatively and post-operatively during the long term review process. Results: Thirty-four (79.1%) patients out of the 43 were available for the review with the patients stating there was much (100%) improvement in terms of the pain they felt in the neck, upper and lower back regions. There was no re-occurrence of the condition in any of the patients; 29 (85.3%) stated they were satisfied and that their new breast size(s) was/were excellent for them. Conclusion: The pre-operative complications experienced by these patients were resolved hence this surgical procedure was good and the quality of life of these persons had been improved.
