Breast metastasis from a pulmonary adenocarcinoma: Case report and review of the literature

Abstract: Abstract. Breast metastasis from extra-mammary malignancy is rare. An incidence of 0.4-1.3% has been reported in the literature. The primary malignancies most commonly metastasizing to the breast are leukemia, lymphoma and malignant melanoma. We present a case of metastasis to the breast from a pulmonary adenocarcinoma, diagnosed concomitantly with the primary tumor. A 43-year-old female presented with dyspnea and a dry cough of 3 weeks' duration. A subsequent chest radiograph revealed a massive pleural effusi… Show more

“…Some studies have shown that on mammography and conventional US, the two (metastatic and primary breast cancer) are indistinguishable from one another. 4,17 Sonoelastography has been used to aid in diagnosis of malignant breast lesions. Primary breast cancer and other metastatic disease to the breast are most often more stiff than the surrounding tissue on sonoelastography.…”

“…A 77% of patients, according to Lee et al ,26 had a history of extramammary primary carcinoma and the mean time from diagnosis of the primary cancer to breast metastasis was 13.9 months. A review of literature revealed just 5 of the 20 patients with lung adenocarcinoma to have a synchronous presentation, both metastatic breast deposits and primary lung cancer presenting simultaneously 6 22 24 27. The American Joint Committee on Cancer (AJCC) defines inflammatory breast cancer as ‘a clinical–pathological entity characterised by diffuse erythema and oedema (peau d'orange) involving a third or more of the skin of the breast’ 28.…”

“…Extensive surgery with curative intent is less likely to be successful when performed in cancers in later stages, especially metastatic disease, and is associated with higher mortality and morbidity. Five of the seven patients with breast metastasis from lung adenocarcinoma who underwent surgery had synchronous breast metastasis 6 22 24 27. The conventional modalities of treatment for metastatic breast disease are systemic treatment or palliative care (which may involve radiation).…”

“…TTF-1 was positive in 93% of SCLCs and negative in breast adenocarcinomas (15,16). According to the literature, 2,8% of primary breast carcinomas can show weak or focal expression of TTF-1, and those cases are usually associated with CK5/6 or P-cadherin positivity, grade 3 tumours and the 'triple negative' (ER-, PR-and HER2-negative) phenotype.…”
