Bronchoalveolar Lavage Cellular Analysis and Gallium Lung Scan in the Assessment of Patients with Amiodarone-Induced Pneumonitis

Abstract: Seven patients suffering from amiodarone-induced pneumonitis were followed after diagnosis over a period of 16 ± 6 months. All of them showed clinical, radiographic and functional manifestations of interstitial lung disease. Bronchoalveolar lavage was performed on 6 of them and disclosed lymphocytosis in 2 cases (associated to eosinophilia in 1 of them), neutrophilia in 1 and normal differential count in 2. In 1 case there were abundant hemosiderin-laden macrophages. In addition, all patients evidenced an incr… Show more

“…The quantified lung activity was then compared to the hepatic activity and a lung-hepatic index (LHI) obtained. The uptake of gallium-67 in the pulmonary parenchyma was considered abnormally elevated when the LHI was higher than 0.25 [27].…”

“…Gallium scanning has been used extensively in the staging of interstitial lung disease (Keogh et al 1983;Klech Offprint requests to: M. Yeates, Department of Nuclear Medicine, Royal Prince Alfred Hospital, Camperdown, NSW, Australia * This work was supported in part by the Australian NH&MRC et al 1987;Xaubet et al 1987;Gibson et al 1988;Pantin et al 1988;Sfakianikas et al 1988;Breit et al 1989) with quantitation of the degree of pulmonary gallium uptake being shown to be useful in some studies (Bisson etal. 1983(Bisson etal.…”

“…The BAL in patients with APT can show an increase in the number of neutrophils, CD8 ϩ lymphocytes and the presence of foamy cells with lamellar inclusions. 17 Biopsy findings include septal thickening, interstitial edema and lipids within endothelial cells and the interstitium. 18,19 Other pathologic findings include acute organizing pneumonia, Type II cell hyperplasia and free foamy intra-alveolar macrophages.…”
