Bruikbaarheid en validiteit van de Nederlandse versie van de Montreal Cognitive Assessment (MoCA-D) bij het diagnosticeren van Mild Cognitive Impairment

Abstract: The MoCA-D distinguishes between healthy elderly, MCI patients and dementia patients. However, in this study, insufficient sensitivity and poor specificity were found. For the present, applying a broader and flexible screening procedure in order to detect MCI seems a more useful method than the interpretation of one test result in particular.

“…The clinical situation of detecting CI (MD + MCI) below this cutoff had a sensitivity of 95%.A cutoff score for diagnosing dementia is still under debate but is often set around 21,17,18,38 which in our study results in a sensitivity of 90%. “…17 It is important to validate the MoCA in specific settings, as the selection of subjects with different characteristics may influence the test characteristics of a scale such as the MoCA. 17 It is important to validate the MoCA in specific settings, as the selection of subjects with different characteristics may influence the test characteristics of a scale such as the MoCA.…”

“…This screening test takes approximately 10–15 min to administer and consists of 30 items measuring eight cognitive domains. The MoCA is sensitive in differentiating milder forms of cognitive impairments and has been validated in patients with Parkinson’s disease, Huntington’s disease, substance abuse, and mild cognitive impairment (MCI) (Freitas et al 2012 ; Larner 2012 ; Videnovic et al 2010 ; Zadikoff et al 2008 ; Wester et al 2013 ; Thissen et al 2010 ). However, to date, few studies have investigated the validity of the MoCA in HIV-infected patients (Koski et al 2011 ; Overton et al 2013 ; Hasbun et al 2012 ; Milanini et al 2014 ).…”

“…Participants were community-dwelling persons aged 50 years or over recruited from a memory clinic involved in the study or were recruited by further neuropsychological testing of persons aged ≥50 years with memory complaints interested in participating in the study. All participants were screened for cognitive impairments with the MoCA battery using scores >18 and <26 as criteria with 88-100% sensitivity and 94-100% specificity for MCI (adding 1 point for <12 years of education) [20,21]. Subsequently, participants were tested with additional neuropsychological tests to operationalize the clinical MCI criteria described by Petersen [22] (“MCI patients were defined as those having scores typically between 1-1.5 SD below the mean for their age and education matched peers on culturally appropriate normative data” [22]).…”
