Bypass Grafting vs Endovascular Therapy in Patients With Non-Dialysis-Dependent Chronic Kidney Disease and Chronic Limb-Threatening Ischemia (CRITISCH Registry)

Abstract: Purpose: To report the outcomes of bypass grafting (BG) vs endovascular therapy (EVT) in patients with non-dialysis-dependent chronic kidney disease (CKD) and chronic limb-threatening ischemia (CLTI). Materials and Methods: The CRITISCH Registry is a prospective, national, interdisciplinary, multicenter registry evaluating the current practice of all available treatment options in 1200 consecutive CLTI patients. For the purposes of this analysis, only the 337 patients with non-dialysis-dependent CKD t… Show more

“…To date, the available evidence is mainly based on retrospective observational studies and there is a paucity of mid-term outcomes [ 5 , 7 , 11 , 17 , 18 ]. Thus, evidence-based recommendations to guide the therapy of patients with CKD are still lacking while valid practice guidelines also do not contain strong recommendations concerning screening strategies in cohorts at risk [ 4 , 7 , 10 , 19 , 20 , 21 , 22 ]. These gaps in knowledge and the need for further studies on the combination of PAOD and CKD was highlighted just recently in a conference conclusion of the Kidney Disease: Improving Global Outcomes (KDIGO).…”

“…Ultimately, LBP will need to be more closely correlated with clinical outcomes, including rates of wound healing and limb salvage. Stavroulakis et al 12 report on the outcomes of bypass surgery vs endovascular therapy in patients with non-dialysis-dependent CLTI. The authors report that endovascular therapy was associated with improved rates of amputation-free survival compared with bypass grafting.…”
