Camouflage treatment for skeletal Class III patient with facial asymmetry using customized bracket based on CAD/CAM virtual orthodontic system:

Abstract: When considering camouflage orthodontic treatment for Class III malocclusion with skeletal facial asymmetry, it is crucial to preserve the favorable compensated posterior occlusion. Once the inclination of the compensated occlusion is changed during orthodontic treatment, unstable occlusion, such as crossbite or scissor bite may occur. A 23-year-old female patient had anterior spacing with Class III malocclusion and a mandibular asymmetry. A nonsurgical approach was adopted. The treatment objectives were to es… Show more

“…This is the first study to describe a CAD-FR3. So far, CAD/CAM technology has mainly been used for aligners 6 , 7 and fixed orthodontic appliances 10 , 11 ; it has very rarely been described in relation to functional orthopaedic appliances. This might primarily be because it is difficult to fuse the metal elements and plastic base of functional appliances by means of CAD/CAM technology.…”

“…Individual studies show direct printing of aligners 8 , but, to date, clinical implementation does not yet seem sufficiently possible with existing printing materials 9 . More examples include customised brackets 10 , 11 , bonding trays 12 14 , CAD/CAM anchoring appliances 15 and fixed CAD/CAM retainers 16 . Other experimental approaches, such as customised CAD/CAM archwires, have been described in individual studies 17 ; however, their use is not yet commonplace.…”

“…6,7 Previous studies have reported the use of the system for adult camouflage treatment, although not for surgical cases. 8,9 This system may be useful in surgery cases for planning of individualized and efficient presurgical orthodontic treatment and for obtaining predictable postsurgical occlusion. This case report describes the use of the CAD/CAM virtual orthodontic system in a skeletal Class III adult patient undergoing orthognathic surgery and orthodontic treatment with maxillary first premolar extractions.…”

“…Moreover, skeletal Class III treatment difficulty increases considerably when associated with facial asymmetry. The asymmetry usually includes mandible and dental midline shift, canted occlusal plane, unilateral crossbite and different posterior occlusion between the right and the left side [ 7 , 8 ]. Orthognathic surgery is a good option to correct the severe mandibular asymmetry with good result but patients may refuse surgery because of risks, high cost and long recovery time.…”
