Can hysterosalpingo-foam sonography replace hysterosalpingography as first-choice tubal patency test? A randomized non-inferiority trial

Abstract: STUDY QUESTION Does hysterosalpingo-foam sonography (HyFoSy) lead to similar pregnancy outcomes, compared with hysterosalpingography (HSG), as first-choice tubal patency test in infertile couples? SUMMARY ANSWER HyFoSy and HSG produce similar findings in a majority of patients and clinical management based on the results of either HyFoSy or HSG, leads to comparable pregnancy outcomes. HyFoSy is experienced as significantly le… Show more

“…In contrast with HSG, it involves no exposure to radiation and forgoes the use of iodine contrast media, thus permitting safe use by women with hypothyroidism [ 19 , 20 ]. At the same time, it is reportedly less painful and better tolerated by patients [ 7 , 21 ], less costly, and more widely available, as it can be performed within the Fertility Clinic, using already available ultrasonographic equipment.…”

“…Besides cost-effectiveness, we have to consider other aspects of the tests as well. HSG represents a higher burden for both patient and clinician than HyFoSy: it is experienced as significantly more painful (based on Visual Analogue Scale; 5.4 cm versus 3.1 cm; P < 0.001 ( van Welie et al , 2022 )) and causes exposure to ionizing radiation and iodinated contrast medium. HyFoSy can be easily performed by a gynaecologist or fertility doctor allowing a one-stop fertility work-up with no need for the radiology department and staff.…”

“…The FOAM trial was a multicenter prospective, comparative study with a randomized design, performed in the Netherlands between May 2015 and January 2019 ( van Welie et al , 2022 ). In this study, the effectiveness of clinical management based on the results of tubal patency testing by HyFoSy and HSG during fertility work up was compared.…”

“…When discussing the optimal tubal patency test, it is important to take into account that clinical management based on the results of an HSG or HyFoSy does not change the live birth rate. This has been shown in a clinical trial in which women underwent both an HSG and a HyFoSy, in case of discordant results, women were randomized to clinical management based on the results of the HSG or HyFoSy (Welie et al, 2022). Nonetheless, THL has as benefit over HyFoSy in that minimal endometriosis and adhesions can be diagnosed, which significantly reduce the non-IVF conception (fecundity rate ratio of 0.35; 95% CI 0.17-0.71) (van Kessel et al, 2018).…”
