Cancer in the HIV-Infected Host: Epidemiology and Pathogenesis in the Antiretroviral Era

Abstract: Cancer and HIV are inextricably linked. Although the advent of antiretroviral therapy has led to a marked decline in the incidence of malignancies classically linked to immunosuppression (AIDS-defining malignancies, or ADMs), this decrease has been accompanied by a concomitant rise in the incidence of other malignancies (non-AIDS-defining malignancies, or NADMs). Population-based cancer registries provide key information about cancer epidemiology in people living with HIV (PLWH) within resource-rich countries.… Show more

“…HIVIIs are a unique group of patients in the context of HNSCC. They have greater exposure to tobacco and alcohol, have impaired immune function, and consequently are more prone to infection by high‐risk HPV . In this way, these patients have multiple key risk factors for the development of cancer and particularly HNSCC.…”

“…Kaposi sarcoma and non‐Hodgkin lymphoma are the most common neoplasms among HIVIIs and are AIDS‐defining malignancies. However, since the advent of antiretroviral therapy, the incidence of other malignant tumors (non–AIDS‐defining cancer) has increased, and they have become important causes of death for these patients …”

“…This estimate was higher than reported earlier in a meta-analysis of 1 in 377 per year among HIV-infected men with HSIL (5). One of the limitations of this study is that the baseline disease classification was based on cytology, which has been shown to have relatively low sensitivity for anal HSIL and underestimate the severity of anal disease (6). Progression from anal HSIL to cancer was also reported to be high in another study.…”

“…Most studies of the prevalence or incidence of anal HSIL in the post-ART era continue to show a high prevalence of this cancer precursor lesion in HIV-infected men and women (6). These cross-sectional studies suggest that ART has a limited effect on reducing HSIL incidence or inducing HSIL regression.…”

“…The availability of highly active antiretroviral therapy (HAART) has not only reduced morbidity and mortality among people living with HIV (PLWH) but has changed the epidemiology of HIV infection [1,2]. Since the advent of HAART in 1996, the population of PLWH has expanded, with particular growth in the group of patients $40 years old [3].…”
