Cancer Metastatic to the Orbit

Abstract: The most common primary cancers that metastasize to the orbit are breast, prostate gland, and lung cancer. In 19%, there is no history of cancer when the patient presents with ophthalmic symptoms and in 10% the primary site remains obscure despite systemic evaluation. The systemic prognosis is generally poor.

“…In Shields' series, the primary tumour remained obscure despite systemic evaluation in 10% of metastatic orbital tumours. 6 In the current case, immunoperoxidase stains for PSA in orbital lesions and abnormal serum PSA provide us a hint to make an effort to explore the primary prostate carcinoma by thorough evaluations and repeat biopsies. Although the survival rate for patients with metastatic prostate cancer at diagnosis is relatively poor (median survival was 2.5 years), 7 timely intervention based on early diagnosis may improve their prognosis.…”

“…Complications include retinal detachment, uveitis, papilledema and secondary glaucoma. 1,2,8,29 Appropriate investigations require consideration of the patient's clinical status and goals. If the patient has a previously-confirmed cancer diagnosis, it would be reasonable to defer further workup in favour of immediate treatment.…”
