Capsule Endoscopy in the Investigation of Patients with Portal Hypertension and Anemia

Abstract: Varices, diffuse changes of mucosa with inflammatory-like appearance, and angiodysplasias and/or spider angiomas are detected more often in patients with PHT than in controls, and probably constitute the endoscopic characteristics of PHE. CE of the SB added a significant number of likely important findings to those detected by conventional endoscopic techniques for the clinical management of patients with PHT and anemia.

“…Multiple angioectasia or diffuse mucosal changes compatible with portal hypertensive enteropathy [30] were in fact detected in ten of our 13 cirrhotic patients, six of whom had ongoing overt bleeding. Although there may be an argument that the prevalence of portal hypertensive enteropathy in our cirrhotic patients was high, recent articles have reported equivalent values of prevalence, up to 60-70%, of portal hypertensive enteropathy among cirrhotic patients [30][31][32][33]. In addition, it has been suggested that severe cirrhosis is not a sine qua non condition for the occurrence of portal hypertensive enteropathy [31].…”

“…Capsule endoscopy is being used increasingly to investigate gastrointestinal bleeding of obscure origin and disorders of the small bowel with portal hypertension [31][32][33]. In this case, we did not perform capsule endoscopy; however, it may be key to the diagnosis of disorders of the small bowel with portal hypertension.…”

“…In patients with PH and/or LC, the most common indication for CE is obscure gastrointestinal bleeding (OGIB). According to the guidelines of the European Society of Gastrointestinal Endoscopy [21], CE is currently the first-line imaging modality for diagnosing small-bowel lesions [22-24], especially for patients with OGIB. In addition, CE is usually preferred over DBE, as it is a safe and minimally invasive procedure, with the capsule being easy to swallow; yet its detection capabilities are extensive, such that small lesions, such as an angioectasia of the small bowel, can be diagnosed [25, 26].…”
