Características sociodemográficas y clínicas de la sífilis gestacional en Cali, 2018

Abstract

Introducción. La sífilis gestacional se considera un evento de interés en salud pública, debido a las repercusiones que ésta tiene sobre el binomio madre-hijo. En Colombia, pese a tener definidos protocolos para la notificación, diagnóstico y manejo, se sigue evidenciando limitaciones en su control.
 Objetivo. Describir las características sociodemográficas, clínicas y la distribución espacial de las pacientes con sífilis gestacional en Cali, Colombia, año 2018.
 Materiales y métodos. Estudio transversal que incluyó 427 casos de sífilis gestacional reportados al Sistema de Vigilancia en Salud Pública (SIVIGILA). Procesamiento estadístico utilizando programa R versión 3.5.3, variables cualitativas presentadas como proporciones y cuantitativas a través de medidas de tendencia central y dispersión. Distribución espacial usando programa Qgis 3.0.
 Resultados. La razón de sífilis gestacional correspondió a 17 casos por mil nacidos vivos y muertos. 57.1% régimen subsidiado y 16.6% población no asegurada. Se diagnosticaron durante el embarazo 90.4%. 47.2% recibió tres dosis de penicilina, el 57.6% de los contactos fueron tratados. Conclusiones. La tasa de sífilis gestacional en Cali, 2018, fue superior a la nacional, con mayor frecuencia en gestantes en condición de vulnerabilidad socioeconómica, consistente con la distribución espacial en general. Se evidencia falta de oportunidad en la detección temprana y el manejo de la infección, tanto en las gestantes, como en sus contactos, lo cual dificulta el control de la enfermedad y refleja inadecuada aplicación de la Ruta Integral de Atención en Salud Materno-perinatal.
