Carcinoid heart disease: a guide for screening and timing of surgical intervention

Abstract: The cardiac manifestations of a neuroendocrine tumour are referred to as carcinoid heart disease (CaHD) and are associated with a poor prognosis. Surgical intervention is the only proven therapeutic option and may prolong survival and quality of life. No consensus has been reached internationally with regard to screening for CaHD and the optimal timing for surgery. Although limited evidence is available on this matter, a trend towards early surgery and subsequent reduced mortality has been observed. In this re… Show more

“…Surgical resection of the primary tumor and resection of liver metastases appear to decrease the cardiac progression in CaHD and improve prognosis [ 16 ]. Since hepatic surgery exposes a patient to the risk of perioperative bleeding, hepatic intra-arterial treatment (transarterial chemoembolization, selective internal radiotherapy) is well suited to patients with hepatic metastases [ 21 ].…”

“…This involves secretion of vasoactive substances, leading to plaque-like deposits of fibrous tissue on cardiac valve cusps and leaflets. Our patient had a nonfunctioning NET, negative for all biomarkers including 5-hydroxyindoleacetic acid (5-HIAA) and chromogranin [ 4 - 5 ]. Congestive heart failure was due to intra-cardiac metastasis by the tumor.…”

“…Valve replacement surgery or valvuloplasty is the only definitive treatment and should be planned carefully and early in the course of the disease [76,85]. e choice of valve prosthesis is still under debate: bioprosthesis is preferred to mechanical valves, due to the absence of need for anticoagulation and the risk of bleeding complications, but these valves may be less durable and more vulnerable for the recurrence of disease.…”
