Carcinoid metastasis to extraocular muscles: case reports and review of the literature

Abstract: Carcinoid tumors are slow growing, low-grade malignant neoplasms that are believed to originate from neuroendocrine cells, usually in the gastrointestinal mucosa. Metastasis of carcinoid tumor to the orbit is a rare occurrence. When metastasis does occur, the choroid is the most common ocular structure involved. We report two cases of unique involvement of extraocular muscles.

“…Seventy‐five percent of those treated reported symptomatic improvement in our pooled data which matches the literature for orbital metastases . The growing role of radiotherapy in orbital tumours has been described by Finger and Kim et al ., and this is particularly applicable to EOM metastases where some primary tumours, such as carcinoid, are very radiosensitive . Ophthalmic side effects of radiation include optic neuropathy, corneal scarring and radiation‐induced retinopathy .…”

“…To our knowledge, there have only been two cases reported in the literature where metastasis has occurred in subcutaneous tissue, as the first clinical manifestation of ileal carcinoid 8,9 . There have also been a few case reports of carcinoid metastases to orbital structures including the choroid, uvea and extraocular muscles, suggesting a relative predilection for carcinoid metastases to these sites 10−13 . Excepting these cases, to our knowledge there has been no previous report of carcinoid tumour metastasizing to skeletal muscle.…”

“…The most common manifestation is a solitary, unilateral tumour, although metastases may rarely be multiple and bilateral. Orbital metastases generally involve the choroid rather than orbital structures (8), the most likely explanation being the high blood flow of the choroids (9). For unknown reasons, the majority of uveal metastases originates in a primary bronchial carcinoid, whereas the majority of extraocular orbital metastases arises from primary tumours of the ileum (6).…”
