Carcinoma arising in thyroglossal remnants

Abstract: Three patients with a papillary carcinoma arising in a thyroglossal duct cyst are presented and the literature is reviewed. This rare malignancy is seen mostly in women between the ages of 20 and 50 years. The distribution of carcinoma subtypes differs from that of thyroid carcinomas and thyroglossal duct carcinoma is recognized as a primary tumour. The diagnosis is seldom made pre-operatively though especially in older patients with midline swellings in the neck the diagnosis should be considered. Sistrunk's … Show more

“…Unlike nonneoplastic TGDCs, which show no gender predilection [2][3][4], TGDCCa affect women more frequently than men with a 3:1 female to male ratio observed in the present series, or 2.3:1 overall when including the literature ( Most patients present with a mobile midline neck mass, clinically indistinguishable from benign TGDC [2,13,17,[19][20][21]. The finding of a fixed mass has been emphasized as a possible indicator of malignancy [3,20,36], but was present in only 2 patients in this series.…”

“…TGDCCa are thought to represent primary neoplasms arising from ectopic thyroid gland tissue commonly found in TGDCs [7,9,[17][18][19][20][21]. There is, however, theoretic consideration that these tumors may represent metastases from a primary carcinoma of the thyroid gland [50].…”
