Carcinosarcoma and Pseudosarcoma of the Esophagus: Two Names, One Disease—Comprehensive Review of the Literature

Abstract: In the present review we have collected 127 carcinosarcomas and 56 pseudosarcomas of the esophagus from the literature. There were no significant differences in age, sex, symptoms, location, or stalk involvement between the two groups. Infiltrative lesions were observed in 7.5% and 4.0%, respectively. The rate of lymph node metastasis was 52.5% for carcinosarcoma and 45.0% for pseudosarcoma. The resectability was similar in the two groups, and the recurrence rates were 38.0% and 45.5%, respectively. Although r… Show more

“…9 This theory is supported by the frequent finding of a transition zone between the two cell populations and identical genetic alterations are observed in both components. 11 The second is the collision concept, which hypothesizes that two individual stem cells may undergo malignant transformation (true carcinosarcoma) independently and simultaneously, and are actually separate tumors that have merged. 9 Expression of cytokeratin and vimentin were mutually exclusive in the carcinomatous or sarcomatous elements, which may support the collision concept.…”

“…The third is that the sarcomatous cells are derived from metaplastic epithelial cells. 7,8 Immunohistochemical stain, electron microscopic findings and the common p53 point mutations of both components support the third theory. 3 Most esophageal carcinosarcoma present as large polypoid mass and most frequent symptom is dysphagia.…”
