Cardiac Arrest from Local Anesthetic Toxicity After a Field Block and Transversus Abdominis Plane Block

Abstract: We report the case of a 25-year-old female scheduled for laparoscopic gynecologic surgery under general anesthesia. At the end of laparoscopy, an intraperitoneal infiltration (ropivacaine 0.75%, 20 mL) was administered by the surgeon without informing the anesthesiologist. After tracheal extubation due to significant postoperative pain, the anesthesiologist performed a bilateral transversus abdominis plane block (ropivacaine 0.75%, 40 mL). A seizure followed by ventricular arrhythmia developed 10 minutes after… Show more

“…Several incidents of toxicity in association with TAP block were reported by Naidu and Richebe and Scherrer et al [2930] and others. [3132] Monitoring every patient with TAP blocks is essential for at least 45 min post-injection.…”

“…However, these techniques have their specific complications, e.g. needle injuries of nerves causing chronic neuropathic pain, serious local anaesthetic systemic toxicity from infusions of large doses of local anaesthetic drugs [86,87]. Local infiltration analgesia (LIA) -for hip replacement, containing a local anaesthetic and an NSAID has caused renal failure [88].…”

“…The TAP block has remarkably low incidence of complications. Since a TAP block is a field block, where relatively large amounts of local anesthetic are used, especially when performed bilaterally, it is vital to ensure that the total toxic dose of local anesthetic is not exceeded 27. Like any other regional block, it is important to have lipid emulsion and other resuscitation equipment at hand.…”
