Cardiac Enzymes, Renal Failure and Renal Transplantation

Abstract: Diagnostic accuracy of the currently available serum markers of cardiac injury, such as myoglobin, creatine kinase and its myocardial isoform, are altered in patients with renal failure. It is shown that cardiac troponins have decreased diagnostic sensitivity and specificity in patients receiving renal replacement therapy. Data regarding serum levels of these cardiac biomarkers, especially those of the cardiac troponins, in patients with a transplanted kidney are limited. Current data show that levels of cardi… Show more

“…Chronic kidney disease, with its resulting renal dysfunction, is associated with excessive cardiovascular mortality, especially in those patients receiving renal replacement therapy (66 ). Prevalence of CVD in patients with chronic renal failure (CRF) is up to 73% and is responsible for approximately half of all CRF-related deaths (67 ).…”

“…Prevalence of CVD in patients with chronic renal failure (CRF) is up to 73% and is responsible for approximately half of all CRF-related deaths (67 ). In patients with a functioning kidney allograft, CVD is the most common cause of death (66 ). Atypical presentation of ACS (often without angina) and silent ischemia occur with increased frequency in patients with CRF; interpretation of the ECG in these patients can be complicated by the presence of left ventricular hypertrophy, electrolyte derangement, conduction abnormalities, and medications (67 ).…”

“…4,5 Those patients whopresentedlate or with evidence of liverdisease, chronic kidney disease, and those suspected to have pulmonary thromboembolism were excluded from present study. 6 The recruited patients after signing the informed consent underwent a series of investigations such as Electrocardiography, Random blood sugar, Urea, Creatinine, liver function test estimation of cardiac biomarkers such as CK-MB, cardiac Troponin I (cTnI) using electro chemiluminescence immunoassay-2 antibodies sandwich principleand H-FABP by Enzyme Linked Immune Sorbent Assay (ELISA), based on biotin double antibody sandwich technology.…”

“…The reasons for this remain unclear. Possible mechanisms include coronary artery disease, left ventricular dysfunction, left ventricular hypertrophy, cardiac microinfarctions and arrhythmia [6,7]. Non-ischemic myocardial injury due to uremic toxins, may be re-expression of fetal Troponin-T in myopathic skeletal muscles [7].…”

Non-ischemic myocardial injury due to uremic toxins, may be re-expression of fetal Troponin-T in myopathic skeletal muscles [7]. The Food and Drug Administration recently approved the use of cTnT as a biomarker for mortality risk stratification in ESRD, and the use of cTnT for prognostication is also recommended by the Kidney Disease Outcomes Quality Initiative (KDOQI) [8].…”
