Cardiac MRI findings of endomyocardial fibrosis (Loeffler’s endocarditis) in a patient with rheumatoid arthritis

Abstract: Loeffler's endocarditis and cardiac manifestations of the hypereosinophilic syndrome (HES) are rare and difficult to diagnose. We report a case of in a 36 year-old female with a history of rheumatoid arthritis with disabling dyspnea. The transthoracic echocardiogram demonstrated normal systolic cardiac functions and a left ventricular apical thrombus. However, using cardiovascular magnetic resonance (CMR) with inversion-recovery (IR) delayed enhancement, and cine steady-state free precession (SSFP) sequences, … Show more

“…There are numerous case reports of patients with hypereosinophilic syndrome who also suffer from an autoimmune disease including ulcerative colitis, autoimmune hepatitis, autoimmune thyroiditis, multiple sclerosis, systemic lupus erythematosus, antiphospholipid syndrome, myasthenia gravis, and rheumatoid arthritis ( 295 310 ). Some of these patients had more than one autoimmune disease.…”

“…Biopsy also comes with inherent sampling error which affects diagnostic sensitivity. Diagnosis purely on the basis of CMRI findings without endomyocardial biopsy has been reported in patients, and CMRI was able to exclude myocarditis and coronary artery disease [10 , 15 , 20] . CMRI is also a promising modality to follow-up patients to assess response to therapy, as it enables the comparison of serial images [16 , 18] .…”
